Dear Cherry:

I praise God that you have left that life and returned to God and His Church.

To begin, you need to pray the following to reclaim the ground. Although your sin is forgiven, the consequences can remain, such as attachments and ground that Satan stole from you because of your sin. This prayer renounces those things and reclaims the ground in your life for Christ:

Renunciation Of Satan and Claiming the Full Victory

I claim the full victory that my Lord Jesus Christ won on the Cross for me. Having disarmed the powers and authorities, He made a public spectacle of them, triumphing over them by the cross" (Col. 2:15) His victory for my is my victory.

In the name of the Lord Jesus Christ I renounce all the workings of Satan in my life in all its forms, whether brought into my life by my actions or by others. I break all attachments, ground, curses, spells, and rights Satan may have in my life whether such ground was gained through my actions or through others. Strengthened by the intercession of the Immaculate Virgin Mary, Mother of God, of Blessed Michael the Archangel, of the Blessed Apostles Peter and Paul, and all the Saints and Angels of Heaven, and powerful in the holy authority of the name of the Lord Jesus Christ, I ask you Lord to command Satan and all his minions, whomever they may be, to get out of my life and stay out. With that authority I now take back the ground in my life gained by Satan through my sins. I reclaim this ground and my life for Christ. I now dedicate myself to the Lord Jesus Christ; I belong to Him alone. Amen.

Holy Michael the Archangel, defend us in battle. Be our protection against the wickedness and snares of the devil. May God rebuke him, we humbly pray; and do thou, O prince of the heavenly hosts, by the power of God, thrust into hell Satan and all the other evil spirits who prowl through the world seeking the ruin of souls. Amen.

Now as to the counter-top: if you painted or drew any symbols upon it you need to remove those symbols and markings with whatever solution that will work -- turpentine maybe. If you carved any occult markings into the counter-top, then you might sand the counter-top to remove those carvings and repaint or resurface the counter-top. After doing all this, then the following should be done...

If none of this applies, and you just used the counter-top as an altar without defacing it in any way, then go straight to the following...

Using Holy Water, bless the counter-top with the sign of the Cross, in the Name of the Father, and of the Son, and of the Holy Spirit.

Then pray this prayer to bless the counter-top:

Almighty and everlasting God, you made us stewards over the created world, so that in all things we might honor the demands of charity. Graciously hear my prayer, that you blessing may come this counter-top. Cast away, oh Lord, any evil spirit that may be attached to it. Cleanse this counter-top from the stain of sin and from any curse that was placed there because of my sin of Voodoo and Witchcraft. Bless all those who use this counter-top for their needs. Let them always see you as the good surpassing every good and love their neighbor with upright hearts.

We ask this through Christ our Lord. Amen.

Then, since you used your home for witchcraft you need to bless your house with a House Cleansing and Blessing (exorcism) (pdf file). If a priest will do this that is great.  If a priest will not do this House Cleansing maybe he will do a regular blessing, and then you can do the House Cleansing yourself, or I can lead you over the phone to do the blessing.
