Question:

may my spouse who has severe dementia or alzheimer's disease receive holy communion without prior confession ? She is a Catholic.

Dear Lap:

Confession is needed before receiving the Eucharist only if one has committed a grave sin since last confession. Under severe dementia or Alzheimer's disease it is unlikely that your spouse will be responsible for any sins committed.

But, you need to talk with the pastor who knows the situation, or can come to know the situation personally.
